What is Remote Patient Monitoring? Accuhealth's Speciality 

Before we get into the weeds, let’s recap. 

Remote patient monitoring (RPM) involves using devices to transmit a range of health data (i.e. Blood Pressure, Weight, Pulse, Blood Glucose, and more) back to providers via the Electronic Health Record (EHR). When paired with high-quality remote patient monitoring software, it’s an extremely beneficial service. 

Consider a cardiology patient who has to travel hours to get to the doctor. No matter how big and impactful the health system is, that distance can and will negatively affect that patient's healthcare experience.

Instead of taking that long, involved drive, they can be comfortable in their own home and get the accurate care they deserve. It’s a win all around, especially when you consider how much room that opens up in any sized healthcare facility for new patients.

Now, where does chronic care management (CCM) come in? Aren’t they the same thing?
