The process for most remote patient monitoring programs is the same:
1. Accuhealth is integrated.
2. We combine your patient base to find eligible patients.
3. We reach out to, connect with, and onboard those patients while handling the shipping of devices and all relevant patient education.
4. You define what you consider a red flag, and we use algorithms to flag any readings that fall inside of those parameters.
5. Our in-house clinical staff follows up on any abnormal readings, and at the end of each month, your patient’s vital report is automatically uploaded into the AcumenEpic Connect EHR system.

Yes, you can order patient devices directly within your EHR.
First, you’ll go to your Preference List and then make your way to the Miscellaneous section, where providers can search and select “Accuhealth” in the Order Depot.
Once the order is placed, healthcare providers can set up specific ranges for patient vitals. If readings fall outside the pre-defined range, an alert is automatically sent to the provider’s In Basket for timely intervention.
Once you are done filling in the patient-relevant parameters, click Accept.
And finally, click Sign Orders.
As data is received, it will appear in the patient’s chart under the Flowsheet activity. You can see an example below.
A major part of our promise is the patient vitals report, which AcumenEpic users can find via the Incoming Patient Vitals Report interface. These are uploaded automatically at the end of each month.
Click Chart Review and then Media, where you can use filters to choose your Document Type and find the Accuhealth Summary.
