The process for most remote patient monitoring programs is the same:

1. Accuhealth is integrated. 

2. We combine your patient base to find eligible patients. 

3. We reach out to, connect with, and onboard those patients while handling the shipping of devices and all relevant patient education.

4. You define what you consider a red flag, and we use algorithms to flag any readings that fall inside of those parameters. 

5. Our in-house clinical staff follows up on any abnormal readings, and at the end of each month, your patient’s vital report is automatically uploaded into the AcumenEpic Connect EHR system.

But let’s get into the details of how it all works together. 

How AcumenEpic Connect & Accuhealth Work Together, 4 Parts

Below, we review each part of the process. Specifically, we will cover the following: 

  • Placing the order for a device to be sent to the patient as well as setting parameters for that patient
  • Reading the patient’s flow sheet
  • Finding the patient’s vital report

1. Placing the Order

Yes, you can order patient devices directly within your EHR. 

First, you’ll go to your Preference List and then make your way to the Miscellaneous section, where providers can search and selectAccuhealth” in the Order Depot. 

Once the order is placed, healthcare providers can set up specific ranges for patient vitals. If readings fall outside the pre-defined range, an alert is automatically sent to the provider’s In Basket for timely intervention.

Once you are done filling in the patient-relevant parameters, click Accept. 

And finally, click Sign Orders.

2. Tracking Patient Data Through Epic’s Flowsheet

As data is received, it will appear in the patient’s chart under the Flowsheet activity. You can see an example below.

3. Patient Vitals Report via The Interface

A major part of our promise is the patient vitals report, which AcumenEpic users can find via the Incoming Patient Vitals Report interface. These are uploaded automatically at the end of each month. 

Click Chart Review and then Media, where you can use filters to choose your Document Type and find the Accuhealth Summary.

The report consolidates the patient’s vital data, making it simple for healthcare teams to evaluate the patient's health trends over time.
